Buttock augmentation is a surgical procedure that improves the shape of the buttocks, providing a lifted and enhanced buttock profile. The most commonly used methods for augmenting and reshaping the buttocks are fat grafting using one's own fat or synthetic buttock implants.
You may choose to undergo buttock augmentation if your buttocks are too small in proportion to the rest of your body and you desire greater harmony in your body contour. You may also consider buttock augmentation if your buttocks are too flat or have a square shape, and you desire more curves and a more youthful appearance. It is common for significant weight loss or natural aging to leave the buttocks more saggy or even drooping.
How is a buttock augmentation performed?
It has already been mentioned that there are two methods for performing a buttock augmentation, and it will be your plastic surgeon who, based on the characteristics of your issue, your desires, and their experience, will decide which one is better for you.
- Autologous fat transfer
In this surgical procedure, the plastic surgeon will obtain your own fat from one or multiple areas of your body and use it to enhance and reshape your buttocks. Fat harvesting is done through liposuction techniques in various areas such as the back, abdomen, thighs, hips, etc. The harvested fat is processed, removing impurities and other damaged cells, preparing it for transfer using syringes connected to thinner cannulas, and through small incisions that can be easily hidden with clothing. The fat is grafted in multiple layers to ensure a smooth deposit without irregularities or bulges, aiming to reshape and increase the buttock area.
Your plastic surgeon may leave drains and use compression garments or a girdle after surgery.
- Buttock implants
Implants are more suitable when addressing overly flat or poorly defined buttocks, and in patients with minimal available fatty tissue for fat transfer. However, they are not very effective for treating sagging buttocks after significant weight loss. Similar to breast augmentation, a soft silicone implant is placed through a hidden incision in the natural crease between the two buttocks, or through the incision that your plastic surgeon deems best for you, either in the upper-outer corners of the buttocks or in the lower buttock fold. The implants are positioned in a pocket beneath the gluteal muscle and above the pelvic bones. Incisions are closed with sutures, and drains are inserted. Afterward, you will wear a compression garment or girdle.

Care and recovery
Your plastic surgeon will inform you about how much time you will need to return to your normal activities and work. After the surgery, you will receive detailed instructions on the measures to take and the necessary medication, normal symptoms you may experience, and possible signs of complications.
The recovery period is a bit longer and more painful in the buttock area, and you should not sit during the first ten days. Afterward, you can sit with a soft pillow under your thighs. After two weeks, you can sit with a pillow for about 8 weeks. You should walk for a few minutes several times a day to avoid complications in the veins of the lower limbs.

The important complications of buttock augmentation surgery are infrequent.
All surgical procedures carry some degree of risk. Some possible complications of all surgeries include adverse reactions to anesthesia, hematoma or seroma, infection and bleeding, changes in skin sensation, poor scarring, allergic reactions, damage to underlying structures, and unsatisfactory results that may require additional surgeries.
Other specific risks include:
- Fat Transfer:
Fat absorption can be up to 40%, leading to asymmetry. This makes the procedure more challenging to predict, and one or more fat grafting sessions over time may be necessary to achieve the desired “look.”
- Implants:
Implant displacement, causing asymmetry. Capsular contracture or hardening of the scar surrounding the implant. Chronic pain due to the presence of implants. Implant rupture. Any complication or change in shape may require additional surgery.
